Requirements for Admission

Senior High School Requirements

For New Students:
✔ Original High School Card (Form 138)
✔ Certificate of Good Moral Character
✔ Original Copy of NSO / PSA
✔ Certificate of Completion / Diploma (Original and Photocopy)
✔ One 1×1 and Three 2×2 ID Picture (preferably white background)

For Transferees:
✔ Certificate of Honorable Dismissal
✔ Certified True Copy of Grades/TOR (for reference and evaluation purposes)
✔ Certificate of Good Moral Character
✔ Original Copy of NSO / PSA
✔ One 1×1 and Three 2×2 ID Picture (preferably white background)

For Returnee:
✔ Certificate of Honorable Dismissal (if transferred from another school)
✔ Certified True Copy of Grades/TOR (for reference and evaluation purposes)
✔ Certificate of Good Moral Character

PROCEDURE FOR ADMISSION

Fill out the Student Application Form at the Admission Office.
Pay the Assessment/ Entrance Examination at the Cashier’s Office.
Take the Assessment/Entrance Examination at the Student Welfare and Development Office.
If the applicant passed the Assessment/Entrance Examination, he will get his Certificate of Admission and undergo an interview.
Requirements will be submitted to the Student Welfare and Development Office and will be provided with the Admission Slip for enrollment.
